Status Review
This form is provided on the eForms Web site for information only. NRCS conducts a status review on the contract or
agreement. The program participant may or may not participate in the documentation process. Following completion
of the status review and appropriate documentation, the program participant and NRCS sign the form. This form is
applicable to the following programs: Agricultural Conservation Program (ACP), Agricultural Management
Assistance (AMA), Colorado Salinity Control Program (CRSCP), Conservation Reserve Program (CRP),
Environmental Quality Incentives Program (EQIP), Emergency Watershed Program (EWP), Forestry Incentives
Program (FIP), Great Plains Conservation Program (GPCP), Interim Environmental Quality Incentives Program
(IEQIP), Resource Conservation and Development Program (RC&D), Rural Abandoned Mine Program (RAMP),
Rural Clean Water Program (RCWP), Soil and Water Conservation Assistance (SWCA), Water Bank Program
(WBP), Water Quality Incentives Program (WQIP), Wetlands Reserve Program (WRP), and Wildlife Habitat
Incentives Program (WHIP).
Submit the original of the completed form in hard copy or facsimile to the appropriate NRCS servicing office. This
form may not be electronically submitted at this time.
NRCS completes Items 1-9a. The program participant completes items 10 and 10a.
