1. Submit Application Material    
  1. Submit completed and signed CCC-1200-Application and Appendix to local NRCS, FSA, or SWCD office.

    • Form HI-SCIMS Applicant Info. for USDA Programs
    • Concurrence Letter (if applicable)
    • Proof of Control of Land (e.g., Lease, Deed)
    • CCC-501A Member’s Information (if applicable)
    • AD-1026 Highly Erodible Land & Wetland Conservation Cert.
    • CCC-526 Payment Eligibility Avg. Adj. Gross Income Cert.
    • Signature Authority (if applicable)
    • FSA-237 Fax Signature Authorization & Verification (optional)

  1. Date stamp CCC-1200, Appendix, and all other application material. Check Application Material for completeness as soon as it is received. Distribute originals & copies as described in Table 1 of MOU between FSA & NRCS.

3. Rank & Select Eligible Applicants    
  1. Complete Application Evaluation Worksheet, input practice information into Toolkit, and upload practice information from Toolkit into ProTracts to develop Form AD-1155.

  1. Review, initial, and date Application Evaluation Worksheet then e-mail to State Office for 2nd level review.

Send Certified Return Receipt Requested letter to preapproved applicants giving them 15 calendar days from receipt of letter to designate “decisionmaker” and percent shares; provide direct deposit information; and sign Contract (CCC-1200) with Appendix, and Schedule of Operations (AD-1155E) and provide all to NRCS.
