REVISION OF PLAN OR SCHEDULE OF OPERATIONS OR MODIFICATION OF A CONTRACT

This form is provided on the eForms Web site for information only. NRCS provides a summary of the revised conservation plan schedule and cost outlays. Program participant reviews the summary information and signs the form. NRCS and a conservation district official must also sign the form. This form is applicable to the following programs: Agricultural Conservation Program (ACP), Agricultural Management Assistance (AMA), Conservation Reserve Program (CRP), Colorado Salinity Control Program (CRSCP), Emergency Watershed Program (EWP), Forestry Incentives Program (FIP), Great Plains Conservation Program (GPCP), Interim Environmental Quality Incentives Program(IEQIP), Rural Abandoned Mine Program (RAMP), Soil and Water Conservation Assistance (SWCA),Watershed Protection and Flood Prevention Program (WPFPP), Water Bank Program (WBP), Wildlife Habitat Incentives Program (WHIP), and Wetlands Reserve Program (WRP).

NRCS completes Items 1-32 and item 34; participant reviews items included in these blocks and completes item 33.

This item is completed by the program participant. All program participants must sign the form. This is a signature field for the program participant. By signing, the participant acknowledges receipt of this conservation plan including form AD-1155 and agrees to comply with the terms and conditions hereof. Manually enter your signature. If there is more than one program participant, complete items 32C and 32E if applicable.

33D and 33F if applicable
Enter the date you sign the agreement. Enter the date using the format of two digits for the month and day, and four digits for the year, separated by dashes. Example: mm-dd-yyyy. If there is more than one program participant, complete items 32D and 32F if applicable.

Item 34 is for NRCS use only. The NRCS signature constitutes certification that the scheduled contract or agreement items meet plan objectives.
